Reporting

Call 911 for emergency medical assistance if you need urgent medical care. If you are able, notify Lab Staff as soon as possible, thereafter.

While all incidents should be reported to us within 24 hours of occurrence, we are also interested in hearing from persons who are concerned about health and safety associated with making at the GSD, barriers related to accessing Fabrication resources at the GSD, and other relevant feedback.

Incidents include:

  • fire (including a small flare-up that was extinguished, and smoldering)
  • injury (no matter how severe, nor where it occurred on campus)
  • near-miss (could have resulted in a fire or injury)
  • illness due to exposure to hazardous conditions
  • chemical spill or other release

As necessary, we will follow-up with individuals to obtain additional information related to the report.

Information collected is intended to improve the resources provided to the GSD community in support of making, and not as a means of pursuing policy violations against any particular individual. However, if a particular community member is identified as putting the health and safety of our community and environment at significant risk, they may be subject to further investigation and consequences.
